What do I Need?
- Referral from you physician authorizing your need for physical therapy.
- Insurance information card.
- Initial personal / health information.
- Download & complete the Patient Registration forms.
How do I Dress?
Wear something comfortable that allows movement. Shorts are recommended if you have hurt your hip, knee, or ankle. Bring your clothing and gym bag if you wish to utilize our changing room.
